3. Have you ever been convicted of an ethics violation, a felony, or a crime of moral turpitude (such as fraud, bribery, false statements, etc.)?

4. Have you ever been convicted of a drug or alcohol related offense?

5. Have you ever been criminally charged with a domestic violence offense? Or an offence involving racial or sexual harassment? If so, please explain.

6. Have you ever been fired from a job for any of the following reasons? Sexual Harassment; Discrimination of any kind; Use of drugs or alcohol. If so, please explain.

7. Have you served in the Military? If yes, please provide official record documenting your service such as a DD-214, ATTRS Records, or SRB/ARB to endorsements@skagitdemocrats.org.

8. Is there anything in your background that may present a challenge in your ability to run an effective campaign? If so, please explain.

Answering "yes" to any of the above questions will not necessarily disqualify a candidate for endorsement by the Skagit County Democrats. All Democrats running for public office are encouraged to apply for endorsement.

your prior experience relevant to the position,
why you decided to run for this position,
what are the most pressing issues facing the community/district/board you will be representing if elected,
what are the issues you are most concerned about, what solutions do you propose to address these issues, and your previous involvement, if any, in addressing these issues- whether in an elected or appointed position or as a volunteer,
your involvement, if any, in promoting diversity in your workplace, in organizations in which you are a member, etc.,
your position on transparency and public accountability in government, and
how you will wage a winning campaign;
List three of your most significant accomplishments that you made in your most recent term as an elected official or, if never elected, your three most significant accomplishments.
